Medical Consent & Telehealth Disclosure

1. Telehealth Consent
By submitting a health intake form through pepta, you consent to receive healthcare services via telehealth (electronic communications) from a licensed, board-certified physician. You understand and agree to the following:
- Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location.
- Telehealth has limitations, including the inability to perform a physical examination, which may affect the physician's ability to diagnose or treat certain conditions.
- The physician may determine that your condition requires an in-person visit, laboratory testing, or additional evaluation before prescribing treatment.
- You have the right to withdraw consent and discontinue telehealth services at any time without affecting your right to future care.
2. No Guarantee of Prescription
Submitting an intake form does not guarantee that a prescription will be issued. The prescribing physician independently evaluates your health profile and makes all clinical decisions. If the physician determines that a protocol is not appropriate for you, your payment will be fully refunded.
3. Doctor-Patient Relationship
pepta is a technology platform, not a healthcare provider. Use of the pepta website or submission of an intake form does not create a doctor-patient relationship with pepta. A doctor-patient relationship is established only between you and the licensed physician who reviews your intake and accepts you as a patient.
4. Compounded Medications
If prescribed, your medications will be compounded by a licensed US 503A compounding pharmacy. You understand that:
- Compounded drug products are not FDA-approved. The FDA does not verify the safety, effectiveness, or quality of compounded medications.
- Compounded medications are prepared based on a valid prescription from your physician.
- Each shipment includes a Certificate of Analysis (COA) verifying purity and potency.
- Compounded medications may have different characteristics than commercially manufactured products.
5. Risks and Side Effects
All medications carry potential risks and side effects. Common side effects of peptide therapy may include injection site reactions (redness, swelling, discomfort), nausea, headache, dizziness, fatigue, and flushing. Rare but serious side effects are possible. Your prescribing physician will discuss protocol-specific risks during your consultation.
You agree to report any adverse reactions or side effects to your prescribing physician and to pepta support immediately.
6. Shipping and Storage
By providing a shipping address, you authorize pepta and its pharmacy partners to ship prescription medications to that address. You understand that:
- You are responsible for ensuring someone is available to receive shipments that may require signature or temperature-controlled handling.
- Medications must be stored according to the instructions provided with your shipment.
- You will not share, transfer, or sell your prescribed medications to any other person.
7. Accuracy of Information
You certify that all information provided in your health intake form is accurate, complete, and current to the best of your knowledge. Providing false or misleading health information may result in inappropriate treatment and could endanger your health. pepta and its physician partners are not liable for adverse outcomes resulting from inaccurate patient-provided information.
8. Emergency Services Disclaimer
pepta is NOT a replacement for emergency medical services. If you are experiencing a medical emergency, allergic reaction, or life-threatening condition, call 911 or go to your nearest emergency room immediately. Do not use pepta, our chat support, or our telehealth services for emergency medical needs.
9. Communication Consent
By providing your email address, you consent to receive transactional communications related to your protocol, including order confirmations, shipping notifications, refill reminders, and account updates. You may opt out of non-essential marketing communications at any time.
